The on-line athletic eligibility forms packet contains most of the required documents for athletic eligibility. Forms must be completed in their entirety and require the signature of the student-athlete and the parent/guardian. The school athletic director will determine the deadline for the return of these forms at their school. These forms are required for participation in athletics at the school. Note: These forms do not constitute all required proofs/information for athletic participation.
Please note you will need to complete this form in its entirety, before you will be able to submit your registration. You may need a few items before starting this process: insurance cards, student ID #, etc.
Complete each form in their entirety and type in your signature and date where noted. Your electronic signature has the same legal effect and can be enforced in the same way as a written signature and by typing your name to the form you are electronically signing each document.
Print and complete the Pre-participation physical forms.You will need to hand deliver the following forms to your school athletic director.
a) Middle School Pre-Participation Exam Form (Requires signature of physician)
b) Child Nutrition notification letter from CMS child nutrition office (if applicable)
Information you provide must be complete, accurate and truthful. False and/or inaccurate information may result in a 365-day athletic ineligibility period for the student-athlete who signs the forms.
